Jens Axboe dd1145ae4d fs: add fget_many() and fput_many()
commit 091141a42e15fe47ada737f3996b317072afcefb upstream.

Some uses cases repeatedly get and put references to the same file, but
the only exposed interface is doing these one at the time. As each of
these entail an atomic inc or dec on a shared structure, that cost can
add up.

Add fget_many(), which works just l(CR) fget(), except it takes an
argument for how many references to get on the file. Ditto fput_many(),
which can drop an arbitrary number of references to a file.

Jens Axboe 072dfad577 net: split out functions related to registering inflight socket files
commit f4e65870e5cede5ca1ec0006b6c9803994e5f7b8 upstream.

We need this functionality for the io_uring file registration, but
we cannot rely on it since CONFIG_UNIX can be modular. Move the helpers
to a separate file, that's always builtin to the kernel if CONFIG_UNIX is
m/y.

No functional changes in this patch, just moving code around.

Jens Axboe 732fd460bb block: ensure bdi->io_pages is always initialized
[ Upstream commit de1b0ee490eafdf65fac9eef9925391a8369f2dc ]

If a driver leaves the limit settings as the defaults, then we don't
initialize bdi->io_pages. This means that file systems may need to
work around bdi->io_pages == 0, which is somewhat messy.

Initialize the default value just like we do for ->ra_pages.

which is task_numa_work() oopsing on current->mm being NULL.

The task work is queued by task_tick_numa(), which checks if current->mm is
NULL at the time of the call. But this state isn't necessarily persistent,
if the kthread is using use_mm() to temporarily adopt the mm of a task.

Change the task_tick_numa() check to exclude kernel threads in general,
as it doesn't make sense to attempt ot balance for kthreads anyway.

Jens Axboe d062d9826a aio: prevent potential eventfd recursion on poll
commit 01d7a356872eec22ef34a33a5f9cfa917d145468 upstream.

If we have nested or circular eventfd wakeups, then we can deadlock if
we run them inline from our poll waitqueue wakeup handler. It's also
possible to have very long chains of notifications, to the extent where
we could risk blowing the stack.

Check the eventfd recursion count before calling eventfd_signal(). If
it's non-zero, then punt the signaling to async context. This is always
safe, as it takes us out-of-line in terms of stack and locking context.

Jens Axboe eaef83c4c0 eventfd: track eventfd_signal() recursion depth
commit b5e683d5cab8cd433b06ae178621f083cabd4f63 upstream.

eventfd use cases from aio and io_uring can deadlock due to circular
or resursive calling, when eventfd_signal() tries to grab the waitqueue
lock. On top of that, it's also possible to construct notification
chains that are deep enough that we could blow the stack.

Add a percpu counter that tracks the percpu recursion depth, warn if we
exceed it. The counter is also exposed so that users of eventfd_signal()
can do the right thing if it's non-zero in the context where it is
called.

[ Upstream commit 2d7271501720038381d45fb3dcbe4831228fc8cc ]

For passthrough requests, libata-scsi takes what the user passes in
as gospel. This can be problematic if the user fills in the CDB
incorrectly. One example of that is in request sizes. For read/write
commands, the CDB contains fields describing the transfer length of
the request. These should match with the SG_IO header fields, but
libata-scsi currently does no validation of that.

Check that the number of blocks in the CDB for passthrough requests
matches what was mapped into the request. If the CDB asks for more
data then the validated SG_IO header fields, error it.

Jens Axboe 824c212908 bfq: update internal depth state when queue depth changes
commit 77f1e0a52d26242b6c2dba019f6ebebfb9ff701e upstream

A previous commit moved the shallow depth and BFQ depth map calculations
to be done at init time, moving it outside of the hotter IO path. This
potentially causes hangs if the users changes the depth of the scheduler
map, by writing to the 'nr_requests' sysfs file for that device.

Add a blk-mq-sched hook that allows blk-mq to inform the scheduler if
the depth changes, so that the scheduler can update its internal state.

commit c616cbee97aed4bc6178f148a7240206dcdb85a6 upstream.

After the direct dispatch corruption fix, we permanently disallow direct
dispatch of non read/write requests. This works fine off the normal IO
path, as they will be retried like any other failed direct dispatch
request. But for the blk_insert_cloned_request() that only DM uses to
bypass the bottom level scheduler, we always first attempt direct
dispatch. For some types of requests, that's now a permanent failure,
and no amount of retrying will make that succeed. This results in a
livelock.

Instead of making special cases for what we can direct issue, and now
having to deal with DM solving the livelock while still retaining a BUSY
condition feedback loop, always just add a request that has been through
->queue_rq() to the hardware queue dispatch list. These are safe to use
as no merging can take place there. Additionally, if requests do have
prepped data from drivers, we aren't dependent on them not sharing space
in the request structure to safely add them to the IO scheduler lists.

This basically reverts ffe81d45322c and is based on a patch from Ming,
but with the list insert case covered as well.

Jens Axboe 724ff9cbfe blk-mq: fix corruption with direct issue
commit ffe81d45322cc3cb140f0db080a4727ea284661e upstream.

If we attempt a direct issue to a SCSI device, and it returns BUSY, then
we queue the request up normally. However, the SCSI layer may have
already setup SG tables etc for this particular command. If we later
merge with this request, then the old tables are no longer valid. Once
we issue the IO, we only read/write the original part of the request,
not the new state of it.

This causes data corruption, and is most often noticed with the file
system complaining about the just read data being invalid:

[ 235.934465] EXT4-fs error (device sda1): ext4_iget:4831: inode #7142: comm dpkg-query: bad extra_isize 24937 (inode size 256)

because most of it is garbage...

This doesn't happen from the normal issue path, as we will simply defer
the request to the hardware queue dispatch list if we fail. Once it's on
the dispatch list, we never merge with it.

Fix this from the direct issue path by flagging the request as
REQ_NOMERGE so we don't change the size of it before issue.

showing that we never complete that read request. The reason is that
the completion setup is racy - it initializes the completion event

before/during the init. If it does, we are passing garbage to
complete() and we may sleep forever waiting for the event to
occur.

Jens Axboe 38cfb5a45e blk-wbt: improve waking of tasks
We have two potential issues:

1) After commit 2887e41b91, we only wake one process at the time when
we finish an IO. We really want to wake up as many tasks as can
queue IO. Before this commit, we woke up everyone, which could cause
   a thundering herd issue.

2) A task can potentially consume two wakeups, causing us to (in
   practice) miss a wakeup.

Fix both by providing our own wakeup function, which stops
__wake_up_common() from waking up more tasks if we fail to get a
queueing token. With the strict ordering we have on the wait list, this
wakes the right tasks and the right amount of tasks.

Jens Axboe 357c120652 mpage: add argument structure for do_mpage_readpage()
Patch series "Submit ->readpages() IO as read-ahead", v4.

The only caller of ->readpages() is from read-ahead, yet we don't submit
IO flagged with REQ_RAHEAD. This means we don't see it in blktrace, for
instance, which is a shame. Additionally, it's preventing further
functional changes in the block layer for deadling with read-ahead more
intelligently. We already make assumptions about ->readpages() just
being for read-ahead in the mpage implementation, using
readahead_gfp_mask(mapping) as out GFP mask of choice.

This small series fixes up mpage_readpages() to submit with REQ_RAHEAD,
which takes care of file systems using mpage_readpages(). The first
patch is a prep patch, that makes do_mpage_readpage() take an argument
structure.

This patch (of 4):

We're currently passing 8 arguments to this function, clean it up a bit
by packing the arguments in an args structure we pass to it.

No intentional functional changes in this patch.

Jens Axboe 1f57f8d442 blk-mq: don't queue more if we get a busy return
Some devices have different queue limits depending on the type of IO. A
classic case is SATA NCQ, where some commands can queue, but others
cannot. If we have NCQ commands inflight and encounter a non-queueable
command, the driver returns busy. Currently we attempt to dispatch more
from the scheduler, if we were able to queue some commands. But for the
case where we ended up stopping due to BUSY, we should not attempt to
retrieve more from the scheduler. If we do, we can get into a situation
where we attempt to queue a non-queueable command, get BUSY, then
successfully retrieve more commands from that scheduler and queue those.
This can repeat forever, starving the non-queuable command indefinitely.

Fix this by NOT attempting to pull more commands from the scheduler, if
we get a BUSY return. This should also be more optimal in terms of
letting requests stay in the scheduler for as long as possible, if we
get a BUSY due to the regular out-of-tags condition.

Jens Axboe 943e942e62 nvme-pci: limit max IO size and segments to avoid high order allocations
nvme requires an sg table allocation for each request. If the request
is large, then the allocation can become quite large. For instance,
with our default software settings of 1280KB IO size, we'll need
10248 bytes of sg table. That turns into a 2nd order allocation,
which we can't always guarantee. If we fail the allocation, blk-mq
will retry it later. But there's no guarantee that we'll EVER be
able to allocate that much contigious memory.

Limit the IO size such that we never need more than a single page
of memory. That's a lot faster and more reliable. Then back that
allocation with a mempool, so that we know we'll always be able
to succeed the allocation at some point.

Jens Axboe 5cb525c831 nvme-pci: handle completions outside of the queue lock
Split the completion of events into a two part process:

1) Reap the events inside the queue lock
2) Complete the events outside the queue lock

Since we never wrap the queue, we can access it locklessly after we've
updated the completion queue head. This patch started off with batching
events on the stack, but with this trick we don't have to. Keith Busch
<keith.busch@intel.com> came up with that idea.

Note that this kills the ->cqe_seen as well. I haven't been able to
trigger any ill effects of this. If we do race with polling every so
often, it should be rare enough NOT to trigger any issues.

Jens Axboe c854ab5773 sbitmap: fix race in wait batch accounting
If we have multiple callers of sbq_wake_up(), we can end up in a
situation where the wait_cnt will continually go more and more
negative. Consider the case where our wake batch is 1, hence
wait_cnt will start out as 1.

wait_cnt == 1

CPU0 CPU1
atomic_dec_return(), cnt == 0
				atomic_dec_return(), cnt == -1
				cmpxchg(-1, 0) (succeeds)
				[wait_cnt now 0]
cmpxchg(0, 1) (fails)

This ends up with wait_cnt being 0, we'll wakeup immediately
next time. Going through the same loop as above again, and
we'll have wait_cnt -1.

For the case where we have a larger wake batch, the only
difference is that the starting point will be higher. We'll
still end up with continually smaller batch wakeups, which
defeats the purpose of the rolling wakeups.

Always reset the wait_cnt to the batch value. Then it doesn't
matter who wins the race. But ensure that whomever does win
the race is the one that increments the ws index and wakes up
our batch count, loser gets to call __sbq_wake_up() again to
account his wakeups towards the next active wait state index.

Bump the internal tag to 32, instead of stealing the last tag in

cannot coexist with NCQ commands.

Jens Axboe 9abd68ef45 nvme: add quirk to force medium priority for SQ creation
Some P3100 drives have a bug where they think WRRU (weighted round robin)
is always enabled, even though the host doesn't set it. Since they think
it's enabled, they also look at the submission queue creation priority. We
used to set that to MEDIUM by default, but that was removed in commit
81c1cd9835. This causes various issues on that drive. Add a quirk to
still set MEDIUM priority for that controller.
